Dear Parent/Guardian,
As part of your child’s requirement for school, a physical examination is needed for those in Pre- Kindergarten, Kindergarten and grades 1, 3, 5 and 7. Health screenings also include a dental health check- up of students in New York State.
New York State also requires a complete and up-to-date immunization record for each child in attendance. All immunizations must be complete in order for your child to start school. Children who are not compliant with the immunization schedule will be excluded on the 14th day of school.
Required immunizations are as follows:
Nursery & Pre-K
• DTaP or DTP: 4 doses
• Polio (IPV/OPV): 3 doses
• MMR (Measles, Mumps & Rubella): 1 dose
• Hepatitis B: 3 doses
• Varicella (Chickenpox): 1 dose
• Haemophilus influenza type b conjugate (Hib): 1-4 doses
• Pneumococcal Conjugate (PCV): 1-4 doses
Grades K-5
DTaP or DTP: 3-4 doses
• Polio (IPV/OPV): 3-4 doses
• MMR (Measles, Mumps & Rubella): 2 doses
• Hepatitis B: 3 doses
• Varicella (Chickenpox): 2 doses
• Haemophilus influenza type b conjugate (Hib): 1-4 doses
• Pneumococcal Conjugate (PCV): 1-4 doses
Grade 6
• All of the above and 1 dose of Tdap (tetanus, diphtheria, and pertussis/whooping cough)
Grade 7 & 8 • All of the above and 1 dose of meningitis vaccine (Meningococcal conjugate)
